Specialist Support Coordination

Specialist Support coordination provides a higher level of support to participants with complex needs, behaviours of concern, medical needs or other high-risk circumstances.

We will support you to address and resolve points of crisis, assist in risk management processes and manage and coordinate complex care team requirements.

We will assist you, your family and carers to identify a broad network of supports that can help you use your plan.

We will work collaboratively with justice, child protection, health, mental health, homelessness services and disability support providers to ensure a consistent delivery of service and access to relevant supports in a crisis or unplanned event.

We will liaise with and report as required to the NDIA and/or associated organisations in relation to you and your plan
